During the initial consultation with our experienced IVF doctor, you will discuss your and your partner's medical history, especially in relation to your 
reproductive health. In the case of an in-person consultation at our clinic, the doctor may perform a general gynaecological examination for the female partner.
Based on this information, the doctor will determine the possible treatment. If necessary, further tests may be recommended to determine the diagnosis and suggest the most effective treatment.
In case of an in-person consultation at the clinic, it is possible to purchase medications directly in Prague. Purchasing medications in the Czech Republic is more cost-effective for patients and reduces unnecessary stress. If patients obtain medications in their home country, we will send the prescriptions electronically and recommend where these can be purchased.

For successful treatment, additional examinations may sometimes be needed. The uniqueness of GENNET lies in the comprehensiveness of services. We provide complex care under one roof while working across various medical fields that play a vital role in the treatment of infertility. This ensures that the IVF treatment process at GENNET is quick and efficient.
However, the examinations can also be conducted in your home country. However, the examinations can also be done in your home country. Based on the consultation with our doctor, your coordinator will send you supporting documents to help you obtain the necessary tests.
Your lead clinician at GENNET will then request the results of tests necessary for treatment from you. They may also recommend/request additional examinations that could help increase the success rate of treatment. You can send us the results via email, and you will only need to come to Prague for the final part of the treatment, which lasts approximately a week.

After diagnosing the patient, the doctor establishes an individual treatment plan and creates a personalised stimulation protocol for the patient.

• Hormonal stimulation typically begins in the first few days of the new menstrual cycle (with the onset of menstruation).
• The ovaries are stimulated using a combination of hormones, which the woman administers according to the stimulation protocol, specifying the exact dosage and timing of medication.
• Hormonal medications may include follicle-stimulating hormone (FSH), luteinising hormone (LH), and sometimes human chorionic
• gonadotropin (hCG)
• The effect of stimulation is then monitored by ultrasound examination. Usually, two ultrasound checks are needed during one cycle of hormonal 
stimulation.

1. You will typically undergo the first ultrasound check-up in your country, and you will send us the results via email.
2. We recommend having the second ultrasound check-up directly at GENNET. Having this examination performed by our doctor using our
equipment helps us maximise the success of the treatment. If you undergo the second ultrasound with us, we will provide the 
necessary preoperative examination for egg retrieval free of charge for you.

• The purpose of hormonal stimulation is to create the optimal number of follicles from which eggs are obtained and used for subsequent in vitro fertilisation.
• Once the follicles reach the desired size, the patient administers a final injection to induce ovulation, which is followed by egg retrieval.
• In case of an in-person consultation at the clinic, it is possible to purchase stimulation medication directly in Prague. Buying the medication in the Czech Republic is more cost-effective and stress-free. If patients obtain medications in their home country, we will send the prescriptions electronically and recommend where these can be purchased.

Approximately 14 days after hormonal stimulation, the follicles reach optimal size for retrieval. 

• The egg retrieval procedure is performed under analgosedation or general anaesthesia, ensuring it is painless and lasts 10 to 15 minutes.
• Due to the general anaesthesia/analgosedation, the patient must undergo preoperative examination (ECG, blood and urine tests, and an examination by an internist). If the patient undergoes the second ultrasound during hormonal stimulation at our clinic, we provide the preoperative examinations free of charge at our clinic.
• OPU is conducted under ultrasound guidance using a special thin needle, which the doctor inserts through the vaginal vault to the ovaries.
• During this short procedure, follicular fluid containing follicles with oocytes (eggs) is aspirated from the ovaries.
• In the laboratory, oocytes are then removed from their follicular shells, and under a microscope, the embryologist evaluates their maturity and quality.
• If for a medical reason, the patient's eggs are not suitable for fertilisation, we recommend considering infertility treatment with an egg donor.
• Egg donation in the Czech Republic is completely anonymous. Patients only receive basic characteristics of the donor.
• From our extensive database, we select the donor based on the physical characteristics of the couple, including blood type and physical appearance:
  • ethnicity
  • skin color
  • eye and hair color
  • height and weight

The primary fertility examination for men is a semen analysis (spermogram). Microscopic examination of semen evaluates the number of sperm in the 
ejaculate, their motility, the shape of their heads, necks, and tails, and overall sperm viability. Through spermogram, we can determine the potential of sperm ́s ability to naturally fertilise an egg.

• In assisted reproduction, the partner's sperm is mostly collected on the day of the partner's egg retrieval.
• If the spermogram results are good, the sperm can be used for fertilisation or frozen for future use.
• In case of deteriorated spermogram values, we recommend visiting a urologist/andrologist.
• If sperm is absent in the ejaculate, it can be resolved through surgical sperm retrieval using TESE or MESA methods.
• Poor spermogram results or inability to obtain viable sperm through microsurgical procedures may lead to the recommendation for infertility 
treatment using sperm donor.
• Donating gametes in the Czech Republic is entirely anonymous. Patients only receive basic characteristics of the donor.
• From our extensive database, we select a donor based on the physical characteristics of the couple, including blood type and physical appearance:
  • ethnicity
  • skin color
  • eye and hair color
  • height and weight

The eggs obtained during egg retrieval are subsequently combined with sperm in a laboratory dish. 
At GENNET, all eggs are routinely fertilised using intracytoplasmic sperm injection (ICSI) method. The egg fertilisation success rate with this method is around 90% and therefore is most commonly utilised. The ICSI method is included in the price of all our treatment packages.
Based on test results or previous treatment cycles, the embryologist/doctor may 
recommend selecting sperm via microfluidic sperm sorting (MFSS) method.

The quality of the embryo is determined by:

• Egg quality
• Sperm quality
• Genetic information of both partners

The more quantitative and qualitative data the embryologist has about the embryos, the better they can assess which embryo is most suitable for transfer.
To determine the embryo quality most accurately our experts take advantage of special laboratory methods:

• Extended cultivation – this method is included in the price of all our treatment packages.
• Permanent monitoring of embryos

Preimplantation genetic testing can also help with selecting a suitable embryo for transfer. In this case, the embryo undergoes genetic testing before being 
transferred to the uterus.
For such genetic embryo testing, we have two methods available:

• PGT- M/SR
• PGT- A

All embryos undergo preimplantation genetic testing at the blastocyst stage (approximately 5-day-old embryos after extended cultivation). After the sample
is collected, the embryo is frozen. Processing of the test results takes approximately 30 days. Upon receiving the results, the patient begins preparation for the transfer of a healthy embryo at the beginning of a new cycle. In the case of a frozen embryo transfer, the patient only needs to visit Prague for 2 to 3 days.

Thanks to the high efficiency of assisted reproduction methods, treatment success, and above all, efforts to minimise risks associated with multiple pregnancies, we transfer only one embryo into the patient's uterus (in exceptional cases, two embryos).
The embryo transfer into the uterus takes place while the patient is fully conscious, and approximately 15 minutes after the procedure, the patient can leave the clinic.
If multiple good-quality embryos are obtained from the treatment cycle, we can preserve these for the future using vitrification (freezing).

In a natural menstrual cycle, the luteal phase begins with ovulation and ends with the next menstruation. In an IVF cycle, this period roughly starts with egg retrieval and continues for the following 14 days. During this time, a woman undergoes embryo transfer, and subsequently, the transferred embryo should implant in her uterus.
To increase the chances of successful embryo implantation, hormonal medication such as progesterone supporting the uterine lining may be prescribed for the patient after embryo transfer.

Approximately 14 days after embryo transfer, the patient undergoes a blood test to determine the level of hCG hormone. The level of this hormone determines whether the patient is pregnant.
